Category 2

For US and Canadian applicants who are non-ASOPRS trained and have been practicing for at least five years 

This pathway is available to board-certified members of the American Academy of Ophthalmology who, in the opinion of the Society, have distinguished themselves in the field of oculofacial plastic surgery.  “Distinction” in the field may include active participation in teaching, publishing, lecturing and advocacy. Category 2 candidates must be at least 5 years out of fellowship training. Upon successful completion of the application process the candidate is inducted into the Society as a Fellow with full voting privileges.

Category 2 Application
As you fill out the online application, you will need the following documents available to upload:

  1. Photo (JPG no larger than 6GB)
  2. CV (PDF)
  3. ABO certificate or equivalent for Canadian applicants (PDF)
  4. Combined into one pdf document:
  • Sum totals of all of the CPT codes related to oculofacial plastic surgery performed as primary surgeon in the last 12 months (from date of application). 
  • De-identified copies of operative reports for each code. In instances where you have performed the procedure numerous times (e.g. blepharoplasty) it is only necessary to send 5 operative reports for this CPT code.
4 Letters of Recommendation must be emailed directly to [email protected] by the writers:
  • Residency Director - recommendation and statement of successful completion
  • Fellowship Director - recommendation and statement of successful completion
  • Two recommendations from current ASOPRS members

Deadline: Category II applications are accepted by March 1 and August 1

Thesis Guidelines

  • You will be invited to submit a thesis after review of your application.
  • Your thesis must be submitted within two years of the invitation.
  • Thesis deadlines are January 31 and May 1. Theses must be submitted by 8 am central on the deadlines and sent to [email protected]

 Examination Guidelines

  • Written and oral board exams are offered every fall the day before the ASOPRS Fall Scientific Symposium.
  • Category 2 candidates are eligible to sit for exams after acceptance of the thesis.

Category 2 Membership Inductions are held during the Spring meeting.
